Well let's see:

1) The Tears suddenly "appeared" some 4,000 years ago,
round-about.  No real explaination as to why.  All
kinds of rumors as to why.

2) The Endless Stair is supposedly linked to Selune,
the moon goddess of Toril.

3) There's a mysterious castle at the heart of the
Tears.  



An idea to tie these things together:

1) The castle at the heart of the Tears has a
connection to the Endless Stair.  Its guarded by a
VERY high level priest of Selune (20+) & his/her
henchmen (all mid-teens or higher in level).  

2) You could almost use a "Maure Castle"-style
adventure here; the guardians are at the "bottom" of
the castle.  Add in a high-level, LE mortal
evil-genius type, who works for the Baatazu.  He's
discovered the Endless Stair, and is trying to get to
it.  His plot is that he'll sneak an army onto it, and
then use it as a "backdoor" into the heart of a major
tanar'ri citadel, achieving a victory for LE & earn
himself bucco power.

3) PCs are contacted by priest to stop the BBG,
obviously.  Maybe he's prevented from acting due to
some restriction, age, or somesuch deal.

4) Toss in a mix of FR organizations (Zhents, Red
Wizards, etc) & PS Factions (Harmonium, Dustmen, etc)
for good measure.  The Tears would act as a
cross-roads for FR, PS, & SJ.

Just a couple of ideas from the top of my head.
