Well, without a common setting to work from, giving it
too much thought will probably just give you a
headache.  ;)

Probably the only way to approach it is through
Planescape & the Planes.  Ala the gods copying one
another theory.  PS does hint in parts that there are
things more ancient than even the gods, with ancient
baatorians and the lost inhabitants of the desert
layer of Arboria (titans of old, perhaps?)  These
ancient beings could have been the wellspring from
which humans, demi-humans, and other races arose. 
Later, gods took the basic design & created their own
versions of these races.  Perhaps humans & elves are
latecomers; the true original race was the
Forerunners, the ancient ancestors of the gith races.

Or not.  Who knows? :)
