Huh, did a quick search myself and didn't turn up
anything. Strange, I thought I had posted about that.
Anyways, here's what I've been using for MotM (it's
pretty close to your chart)

Size Class	Planetary Size	Size
A	Planet: Fine	Less than 10 miles across
B	Planet: Diminutive	10 to 100 miles across
C	Planet: Tiny	100 to 1,000 miles across
D	Planet: Small	1,000 to 4,000 miles across
E	Planet: Medium	4,000 to 10,000 miles across
F	Planet: Large	10,000 to 40,000 miles across
G	Planet: Huge	40,000 to 100,000 miles across
H	Planet: Gargantuan	100,000 to 1,000,000 miles across
I	Planet: Colossal	1,000,000 to 10,000,000 miles
across
J	Planet: Awesome	Greater than 10,000,000 miles across

I always use both, usually as "Size: Medium (E)" or
something like that so that you just need to know one
set to know the size of the moon.
