Yup, nasty place to wind up in! In the hands of a twisted DM, he could
really screw over his PCs.

Maybe, heck, probably. Trick is, since Asylum preys on the poor &
familiness, most people just don't care. It plays a little on the attitude
people from the Middle Ages had about insane people: they locked them in
asylums and pretty much forgot about them.
Add to that the fact that the cultists don't show up too often, since
the operations is spread over literally many hundreds of cities across
scores of spheres, nobody truly appreciates the scope of the operations. As
far as most people are concerned, all it amounts to is a few less crazies on
the streets.

Could just say the open portal to the Abyss causes weird effects, too :)

Somewhat; the 1e MM II gives us cambions (the children of male demons &
female mortals) and alu-fiends (the children of succubi & male mortals).
"Halvies" races beyond these two are largely a 2e invention.
