Maybe Garatoch used to have dozens of volcanic islands on its surface and each volcano was
home to a single dragon. Then there was a dragon war involving powerful magic and most of
the dragons had their island lairs forced underwater. At the end of the battle the two
most powerful dragons were locked in a stalemate. Each living on an island on the opposite
side of the globe. Then in the last battle one dragon created a permanent storm above the
others home and limped back to its lair (the Island of Garatoch to recover).

The dragon could either be sleeping below the surface or might have been driven away by
the arcane.

Maybe it is now helping the scro to survive so that they can help it win its home back.
Maybe it is living in the underdark with the illithids.

Alternatively perhaps the dragon got lonely after hundreds or thousands of years with no
company and did a deal with the arcane to bring the dwarves to its island. It might be
walking around in disguise and be the secret protector of the City of Garotoch. Garatoch
might even be the dragon's name.
