I think part of the problem here is absolutes. There is no answer that is
going to be universal, and in the immortal words of Samuel Clemmens, "No
generalities are true, including this one." There is also the aspect that
each campaign is basically a parallel dimension- they all share a certain SJ
baseline (more of less), but my own Bral is different from yours, night
druids, Paul's etc.
It is only sensible that common courtesies of this nature would have
evolved, it is also sensible to assume that there would be those who eschew
them/are unaware of them/are so alien they do not understand them. This
discussion is trying to put together a logical framework of generalities
that can be customized to any campaign (as all the data we develop is),
whether or not it exists in your SJ universe is, as with all things, the
DM's choice.
Myself, I have little to contribute to this thread but have been following
it with great interest. Please keep going with it!
