The Illithiad is the ultimate source about the Illithid and their society,
if you can grab hold of a copy seize it and read it at any cost.
Ulitharid are Illithid "chiefs" and they sport 6 facial tentacles. In my
campaign, I introduced an avatar of Ilsensine (illithid main god) sporting 8
tentacles.
As for adding tentacles via powers... Illithid think very highly of
themselves - sort of "perfect being" stuff, so I guess they would not alter
their nature.
About tatooing: Illithid communicate via telepathy, so they don't need a
visual mean of recognition, IMO they just "peep" at other flayers' minds to
know their rank.
