there are guards patrolling, as well is in the Donjon (it's the
Donjon, not the palace, that's the most secure part of topside), and
you'd have to do something about the off-duty guards not coming up
"behind" you.

But, that said, it's made explicit that taking topside *would* be
relatively easy, for a powerful/large force.  Problem is, it's the
underside that's the nexus of power, and the real power/leaders would
almost certainly succeed in retreating into the interior caverns and
making their way to the underside.  And those are the *really* secure
parts--even duergar are gonna have a tough time taking those over
(even before the main attack, since they are kept secure, just in
case).
