I once watched a brick building destroyed by flames OVERNIGHT.  No
exaggeration - it was there went I went home for the evening, and in the
morning, there was nothing left but a charred skeleton of part of the
middle, stone retaining wall - and I think only prayers were keeping it
upright.  But still, you do have a point - chances are very good, with
nowhere else to go, that anyone who's been in that house (there was no
one, thankfully) would have been killed by smoke and toxic fume
inhalation (the actual #1 cause of death by fire) or heat (the second,
as opposed to actual physical burns from the flames) before you would
have seen that.

What about the aftermath of a ship fire (which was a big problem before
the advent of ironclads, and remains a fairly serious issue)?  The rules
say that your air remains at whatever level it was fouled by, and that's
a good way of handling it, except that realistically, continued
inhalation of the fumes left by the flames (and the unclearing smoke)
would probably cause death by asphixiation or death by poison within a
few days.  Does anyone know of a way to reflect this in game mechanics?
Or is that feasible without making things nearly impossible for the
players?
