I'm not so certain.
The 3e Manual of the Planes makes it pretty clear that the
cosmology of the Prime Material Plane is the Great Wheel cosmology that we
all know and love.
The 3e Manual of the Planes also makes it pretty clear that any
world that doesn't have that same cosmology is considered an Alternate
Material Plane. And in the canonical 3e Cosmology, Alternate Material
Planes are accessible only via the Plane of Shadow.
The 3e Forgotten Realms Campaign Setting reveals that Toril has its
very own Cosmology separate and distinct from the Great Wheel cosmology of
the Manual of the Planes.
Therefore, Toril is in (or possibly just *is) an Alternate Material
Plane.
Which means that the only way to get from Oerth to Toril is via the
Plane of Shadow.
Which means that, e.g., you can't get there via Spelljamming.
Note that I would love to be proven wrong about this. But seeing
my favorite D&D setting (Planescape) get casually dismembered by the 3e
Manual of the Planes puts me in a pessimistic frame of mind.
