As Richard pointed out, you can just make the
flow into a new transient plane and be done with it.
Or, you could just throw the Phlogiston into the Material
Plane, treat it as an area with no planar access, and
be done with it. In either case, I don't think it really
matters, and shouldn't keep the party's ethereal device
from working (except in the Flow, where it has never
worked anyway). I'm leaning towards "the flow is part
of the prime" theory as the default, with the "the flow is
a transient plane" theory as a fully described and developed
variant. The reason is simplicity -- the DM doesn't have
to worry about a new cosmology every time the PCs enter a
new sphere.

There are several, I think. Off the top of my
head, I'd say check out Ahzad's "Known Crystal Spheres"
(http://www.geocities.com/ahzad/Spheres/Spheres.html).

Overall, I'm more partial to the ships in the original
box than to the ones that came later. They got a bit
whacky for me.
