Well, if it makes you feel any better IMO Wildspace is one of the most
poorly written AD&D adventure modules out there, right up there with
that Vodoni abomination (although Under the Dark Fist at least had a
cool basic storyline if you take the time to make the forces involved
more believable). For the specific example you give from Wildspace, I
agree that it's incredibly unfair to have any instant death scenarios in
*any* adventure where the players have no reasonable way of figuring out
that they might be in danger.
