I think you miss a couple of things here. If you want realism you play games like Rolemaster, and maybe even Shadowrun, where the rules are complicated and really slow down the game. If you want deadly campaigns you play Cthulu or Paranoia (which I hate both of them). AD&D is a game you play to have fun and not worry about realism. Laws are for lawyers not for the AD&D game so I don't stick to them. If I want a PC to kill a opponant quickly the PC will, even if the PC does only 3 damage. If I want an NPC to stay alive then I will do so - the best way of doing that is by having nameless mooks get in front of the most powerful fighters. Mind you, it is amazing how many 20's you roll when you have 40 goblins fighting the party. You might have munchkins, but I found that the powergamer (winger) in my group gets cut down quickly and then goes off, winges, and creates a new character. Seriously he puts to much effort into a one dimensional character that is only going to get killed because all he can say after the battle is "but I didn't get any magic items" Sorry I rave. Anyway, as for bullets, you might be surprised about how durable the human body is. You obviously watch too many movies, but on the news when there is a maniac shooting away in a shopping center, the death rate is quite low. the Port Arthur massacre a couple of years ago in australia resulted in a mortality rate of 37 only because the place was so remote.
