Well, I haven't read the book, but I do gather from the title taht it address
mages, not clerics, and I have read PS, and know that some folks have managed
to get out, so I do that some folks have managed to get out, so I doubt it's
a fool proof system.

Especially when Druids of high level are granted the ability to do so. EAW&F
even has a section on adventuring on the inner planes.

It may be rare, but not impossible.

Also, just because something is hard to do, doesn't mean it isn't a
requirement. Look at the requirements for preservers and defilers to make
their changes....
