Mystara is also surrounded by a "Skyshield" (Poor Wizard's Almanac III).
This prevents the atmosphere from "leaking" into space and also deflects
anything larger than "horse-size".  The only way out is through a
temporary rip caused by meteors or spells.

After 15000' air becomes too thin to breathe, and outside the skyshield
the environment is extremely hostile with no air and extreme cold (1d6
pts damage/turn).  The skyships get around this by the use of create
atmosphere and survival spells.

Also old D&D conversions between spelljammer/airships had gravity in the
D&D universe acting in the "normal" sense (only one up or down on a
ship) instead of the Spelljammer "gravity" enabling someone to walk
upright on the bottom of the ship.

Unless you rule that spelljammers take atmosphere with them because of
the magic of the helm instead of gravity, any spelljammer might be
shocked upon entering this sphere to find all their atmosphere
disappear.

Of course it has always been the wish of the DM in determining what to
keep and what to throw out.  I'm currently running an email campaign in
Karameikos and I've decided to keep the harsh environment rules for
Mystara space.  Only one player is not native to Mystara (the others
have never seen an airship, let alone a spelljammer), and she and her
Spelljamming helm were sent here from Toril via a wild-surge (imagine
the crews' surprise to find the mage and the helm missing).  She is
currently having a ship constructed so she can return, although she
doesn't know about the harsh environment above Mystara yet.  After she
finds out, a few more spell modifications on her ship should make it
space worthy.

One last thing, the old "Champions of Mystara" box set for D&D contains
several skyships, details on constructing skyships, and a page of
conversion rules for Spelljammer/D&D.
