In the Realmspace supplement, it describes the stars (and runes) as being portals to the Semi (Partial But Not Completely) Elemental Plane of Radiance. There's no reason why portals can't exist to other dimensions. I've got a few posts from it archived. (Actually, there were two city discussions) The city with multiple portals to other worlds (or did it travel... I can't remember). We can't blame Those Slimy Rats for copying the discussion for their product -- there have been several multidimensional cities released in the past few years (Nexus, Rifts Atlantis and Worldgate).
